Visual - The interior unit hangs on the wall in ready sight of everyone in the room.
Noise - The interior units typical are not completely silent. Expect more noise than the low level whoosh of air blowing through a diffuser.
Water - You will need to drain condensate away from the inside unit via gravity, or provide a pump (more noise) to pump the condensate to an available drain.
Heat - If you are located in a cold climate you had better plan on installing baseboard heat to supplement the heat pump. The high side wall location of the inside component is great for air conditioning, but really sucks at providing heat at floor level.
Distance from Compressor unit - There is a muximum distance the inside fan/evaporator can be located from the outdoors compressor/condensor unit. Limits are published for maximum horizontal, vertical, and total distance.
Other options to look at include: Magic-Pak (http://www.magic-pak.com) gas-electric self contained thru-wall heating and A/C units - these are popular in small apartments and condos. Or install a self-contained gas-electric unit outdoors (made by York, Trane, Carrier/Day-Night/Payne, etc.) connected to ductwork serving only your new addition. This second option will probably be less costly, gives you options for suppliers and installers, any future service will be performed all outdoors and not in your new space, but may not be practical for a second floor installation.
