Amongst my other job duties I manage 50 Blackberrys & Treos and every single one of them has a Bluetooth headset attached. California is requiring the usage of some sort of hands-free device (wired or wireless) beginning in July 2008, and, well, we wouldn't be very good lawyers if we put our employees into vehicles with cell phones that might cause them to harm themselves or other people... :) We'd hate to have to sue ourselves.
We use a combination of the Plantronics 510, the Jawbone, the Motorola H350, and the Cardo 500.
The Jawbone is the best - head and shoulders above everything else. It pairs with everything, has excellent battery life, and superb audio quality... It's major weaknesses are the replaceable ear hoops which seem very fragile, and the plastic bits that retain the wall charger are not robust. Fortunately Jawbone sells those parts individually, so we just keep some spare parts around. The headset itself has been superbly reliable.
The Plantronics ties with the Cardo for second place - the Plantronics device does not seem terribly long-lived - they develop issues like excess noise or problems pairing and it's all very frustrating. While they work, they work well - better audio quality and better battery life than the Cardo.
I personally like the Cardo - it's a good mix of performance and price. It's what I use, and I have no complaints. It works well with the voice dialing on my Blackberry, which was a big selling point for me. It's cheaper than the Plantronics, has 90% the quality, and 120% the volume (which makes up for the quality deficit :).
The Motorolas are pretty much not useful. We bought them as our cheapies, and honestly I just don't see the value. They're just ok, and cost more than the Cardo, so I've stopped buying them altogether.
I am still not a believer than headsets save lives, or even improve talking-while-driving safety. People who have issues with cell phones while behind the wheel will not improve just because both hands are free. But, it is nice to stick the phone in the glovebox and use Bluetooth to dial the phone, and it's nice to answer a call without fishing for the phone itself. I drive a bunch of 5-speeds, so I need both hands at most times.
