most likely at a store (hotel, restaurant, etc.) and less likely from online purchases. There are card readers out there that can copy all info on your card and then later get duplicated onto a card. The reader is a small hand held device. So image you use the card somewhere and the billing person just swipes your card with his/her reader before/after the actual transaction.
You will not be able to tell how many times your card has been read after you get your card back.
A legitimate online purchase is actually quite safe, unless of course you start out using some fake online sites.
The thief duplicated your card but not necessarily hacked into your account. There are a few more steps, such as your security word to get into your account. So beware if someone calls you/home and ask for your security word for your bank, credit card, or even social security number.
Try to use one card during your travel or websites that you are not too sure off.
