I don't know where you're getting your information, but most medical students do, in fact, go into specialties based, in large measure, on pay. When the "primary care revolution" occurred in the early 1990's, many medical students opted for family practice. Within ten years, that had died down, as HMO's were not compensating family practitioners nearly enough. Orthopaedic surgery and dermatology (just to pick two examples), on the other hand, were highly compensated, so these specialies were, and continue to be, highly "competitive," in terms of applicants for those residency spots. Hence, they take the applicants with the highest scores, grades, and best recommendations, from the best medical schools. It's not that orthopaedic surgery or derm demand the most finely tuned minds, but if you had a large pool of applicants, wouldn't you take only the best?
Now when applying to medical school, prospective applicants do express an interest in specialties based on interest alone. After doing a little math, especially once they know their approximate indebtedness, and after talking to a few doctors in various specialties, their tune changes.
As for the $100 bit, Wasabi correctly pointed out that this is the hourly bill for someone to work on your Saab, typically. I would add that this amount is below the hourly overhead for a typical doctor's office, and also that attorneys typically charge $200 to $300 and hour, CPA's $150 an hour, both of which have much less training than a doctor.
In these days of health care reform, in which reform will occur in the context of a public discussion, the biggest impediment to a meaningful discourse is that most people are so uninformed on the subject that they don't know they're uninformed.
