When you hit the accelerator the rack inside the injector pump goes to the full fuel position until the governor pulls it back when you either get to max RPM or lift your foot. Basically you are just dumping fuel in until the boost/engine speed catches up.
My diesel experience is with Macks about 20 years ago so I'm not up on the latest diesel technology but starting in the late 70s Macks had a puff limiter. it was an air operated plunger screwed into the front of the injection pump that prevented the rack from going all the way to full fuel until the turbo boost built up. as the boost built up the plunger retracted and allowed the rack to go all the way open. I don't know if MB car diesels had any similar devices.
If they smoke at idle they need a tune up. This involves pulling the injectors and rebuilding them, overhauling and calibrating the pump then putting it back together and setting the injection timing correctly. The injectors are basically valves that open at a specified fuel pressure (3000-5000 PSI). If they open prematurely or dribble when they're supposed to be closed the fuel won't atomize properly and will cause excessive smoke.
