1. The cars are crashed into deformable barriers that simulate an identical car. So a small car is tested as if it hit another small car, and large cars against large cars.
2. NHTSA tests are really spot checks rather than full blown test series of different test conditions.
3. Many car companies design the car to do well in tests, rather than real world. There are a number of ways of doing this, and some can actually induce injury.
3a. The dummies are not fully instrumented, so not every significant injury mechanism is recorded. They focus on head injury, but dummies don't have viscera, so bleeding to death from a lacerated liver (for example) isn't measured. (disclaimer: some dummies do have simulated viscera, but they are not the standard dummy used, nor are they instrumented to predict that type of injury). This leads into point 3 above: the manufacturers designing to the test instead of real world crashes.
4. IIHS tests are funded by insurance companies, and are somewhat biased towards repair vs. safety. For example, the cost of auto body repair would factor into the crash test score.
All that being said, crash tests do have their place and can provide valuable data for engineers. It's just that there's a general view out there that crash tests are comprehensive and the end-all of safety evaluations.
