A friend of my wifes had his house built in Ashland Oregon to accomodate his two CarterCars, a 1913 and a 1914. His grandfather had a CarterCar dealership. CarterCars had a constant velocity transmission. From the picture you can see the "flywheel" that spins on a shaft, inline, from the end of the crank and the "wagonwheel" that presses up against it. The wagonwheel shaft is across the axis of the car in the same direction as the chain drive rear axle. You press on the left pedal to push the wagonwheel against the flywheel and you use the lever on the right side of the cockpit to slide the wagonwheel from the center of the flywheel or low gear, to the edge of the flywheel or high(er) gear or to the other side of the center of the flywheel for reverse! You can see the knob on the "gear lever" just inside/above the steering wheel rim at the bottom. This arrangement produced a creeper gear that was very useful for making a CarterCar climb steps and other obstacles in their advertising. I suppose that the drivetrain suffers at higher ratios without the torque multiplication of the lower ratios to improve grip or lessen the strain on the wagonwheel pressing against the flywheel.

I would imagine that the levers in the center of the steering wheel, the inside one setting the spark and the outside one setting the throttle, are easier to modulate when you can control load and rpm's with the transmission without having to shift.
