Turbocharger Failure... a real stumper...

Turbonetics in Cali rebuilt my T3 to a T3/T04E and I installed it on my freshly rebuilt 16VT. 6000 miles later oil is shooting into the turbine housing creating a great smoke screen. So I removed the turbo and sent it back.
Heres what they told me:
1. A "boulder" of some sort came through the feed pipe and clogged it at the turbocharger causing oil starvation and subsequent bearing failure.
or
2. There is some sort of metal particles floating around in my oil which are harder then the bronze used in the turbo thus eating away at the bearing.
According to them it was the worst they had ever seen.
Hers the dilemma
Motor has good compression, runs like a top and makes no unusual noise. Inspection of the oil and filter showed bronze and very small nonmagnetic small silver flakes. In the oil I found two steel shavings that look like they were cut at a machine shop. I had the block bored and rotating assembley balanced as well as some head work done.
So,
Was there a peice of steel left over from the machine work in the oil passages that caused the clog?
Or
Did i forget to put in thrust washers when I put the engine together and kill the bearings with steel shavings?
Any input would be appreciated,
keep in mind that for a sliver of steel large enough to block the turbo
oil feed would have to be sucked up by the pump, go through the filter element(which was intact), and then into the feed line.
