I don't know what car you have, but I have only put WI on my SPG, not the Viggen or 9-5, so I can tell you of my experiences on that car.
Answers:
1) Per Brad's advice (and he pretty much did my install) the water is injected right after the IC.
3.1) Depends upon how you drive. If you always have your foot into it, then you will need one tank of water per tank of gas (assuming your tank is your windshield washer tank or similar size). If you don't drive that aggressively, it can last longer.
3.2) You can use tap water, but you will get deposits on your injector nosel which will need to be cleaned regularly. I run DI water with a little rubbing alchohol mixed in. Brad runs windshield washer fluid because of the methonol. Our windshield washer fluid in TX is too soapy to use.
