The turbocharger sizing will be critical to meet your spool and efficiency requirements. An improperly sized turbocharger will provide similar symptoms as mentioned and the most common culprit is the turbine or turbine housing selection. Although the larger turbine assemblies will provide more shaft torque in conjunction with the larger compressor wheels, they will also attempt to reduce exhaust back-pressures by altering the "catch" of these turbine wheels. Since more exhaust velocity is allowed through a more progressive turbine wheel, less is forced to drive the shaft and there will always be additiona turbo lag. The slower exhaust velocities will also mean less backpressure within the turbine housing at idle and can lead to oil bypassing from the turbine piston seal and into the exhaust.
The turbine A/R ratios are critical to turbocharger response, as well, and the larger ratios will be prone to more lag but reduce exhaust gas backpressure, when needed. Smaller ratios will provide a reduction of turbocharger lag, but will be more restrictive to exhaust velocities.
A camshaft, cylinder head modifications, header, etc. may be of some help in reducing lag, but you may find a more dramatic improvement in the proper sizing of this turbocharger to your engine specifics.
