You can definitely put it back together incorrectly. There are synchro springs that could be put in the wrong place, or the wrong orientation, you can assemble it with the shift selector in the wrong place and not be able to change gears... it's not a like a jigsaw puzzle. With a puzzle, it's obvious when it's correct. With a transmission, not so obvious.
As for how to tell what needs replacing... well, anything broken obviously. There are also specs for measuring synchro wear. I wouldn't take it apart until it doesn't work anymore.
