I have seen and heard about failed tensioner. I replaced an engine a year ago that had bent the valves.
The owner had taken it to the dealer some month earlier and they reset the timing chain tensioner after he complained about rattling noises.
The engine I replaced though had trashed the guides as well and the chain had been riding metal to metal at some places so it's hard to tell how much was broken the first time they reset the tensioner and how much that might have broken later one when the tensioner let go again.
So I don't think resetting the tensioner is the way to go but replacing it instead.
