I replace the small, top most, pulley every 60,000 miles. I replace the tensioner pulley and center idler every 120,000 miles, or thereabouts. The write-up from my recent pulley change is below:

I forgot to include that after you install the center idler, you then install the tensioner, then the top most idler as the final step (prior to installing the belt).
No need to remove the wheel. Just drive the car up on a set of ramps, and have a floor jack with wood block to support the engine from the oil pan when removing the right-side engie mount.
