Stephen and Larry have provided some good information. I'm going to try to expand on that.
Rotor wear is the biggest reason for replacing rotors. This can be checked with a micrometer. Warped rotors cannot be fixed; they require replacement regardless of the level of wear.
Often, when pads are replaced the rotors are also replaced. The reason for this is that the wear on the rotors is so great that less than half of their usable life remains; this means that the rotors will be below their minimum thickness before the new pads have worn out. This means that if only the pads are replaced, it will be necessary to replace only the rotors at a later date.
The usable wear life of a rotor can be measured in mm as the difference between rotor thickness when new and the minimum allowable thickness. When you replace pads, if there is less than half of the rotor's usable wear life remaining it is most expedient to replace the rotors at the same time that the pads are replaced. Rotor replacement should always be done in pairs.
