Monitor the wear every time you rotate the wheels. Somewhere around 4mm replace the pads and rotors. My 2007 9-5 does not have the noise makers. So Saab is now using a more advanced technique just like in the 80's.
I would not swap the outer pads from left to right since they are already seated to the rotors.
