I had this problem once on one of my vehicles. There wasn't anything wrong with the pads or how it was installed. It was the rotor...Yes, the rotor was straight and looked fine, but the hazy glazed surface made the bonding on the new pads to be more difficult.
You can either have your rotors resurfaced or buy new rotors if your rotors are in excellent shape. I think resurface a rotor is about $12.50 each vs $30+ each.
I will bet you that your rotors is the caused of all this.
