There is no need to remove the wheel, but having ramps to get the front of the car higher so you can slide under to properly position the belt is a big help.
Yes, you MUST support the engine with a jack and a block of wood (2x6 or the like to protect the pan) so you can remove the aluminum yoke.
I use an unmodified 8mm Allen wrench to initially loosen the center bolt of the tensioner, then I use pliers on the bolt head to fully remove it.
I have replaced my center idler twice, at 120K miles and 240K miles, so you should still be safe with that one for a while yet.
