It could be the front O2 sensor. The one in my 99 9-5Se 2.3 lpt was on for about 35,000 miles starting about 80,000 miles. A Tech II scan should detect this. After changing the sensor, gas mileage got much better. Average on the SID had been 22mpg. It is now 24.6mpg. The car runs about the same but obviously the cost of the O2 sensor is worth it in the long run. One tech told me that O2 sensors like catalytic converters have a material that when used up makes the O2 sensor useless. The computer senses a defective sensor and runs closed loop which causes a rich mixture. The problems you had will return if not corrected. Also, run Chevron Techron in the gas at least every 5K miles. Check your throttle body and clean it as necessary. Check the PCV system hoses to ensure they are not deteriorating. If so, replace and have the collector box on the back of the engine cleaned out. So much to do and so little time to do it. happy Holidays,
