In a car which has run 20K or more on non-synthetic, I recommend using a good engine flush, such as made by AMSOIL or BG. This will safely clean out any sludge or varnish left in the engine. If you don't, or won't, flush, just change over. The synthetic will clean, more slowly. You might want to change the filter and top off after a couple thousand miles. If you note an increase in rate of oil consumption, change filter and top off.
Having said that, I bought my 88 9000T with about 87,000 miles on it. The shop dropped in a load of AMSOIL without flushing. Drove it a year, 10K, no problems.
Most shops may carry a synthetic, but it is often not one of the top ones. If I couldn't get AMSOIL, I'd probably go with Mobil 1 xW-30.
