a T25 torx bit and a 19mm socket

The torx bit is for the index screw and the socket is for caliper mounting bolts. Start with the index screw -- if you can't get it out, you won't be able to remove the rotor. One of mine cost me two trips to a hardware store. Also get a wire brush, a tube of anti-seize compound and some loctite for caliper bolts.
The detailed rotor replacement instruction is found on the Taliaferro website. There's also plenty of information on this board. Search for rotor replacement topics and for drs<underscore>install in particular.
