The system is supposed to be pressurized. The boiling point is over 100C when under pressure. The cap is supposed to allow the system to pressurize.
If the cap is bad, or you have a leak, the cooling system isn't under pressure, so the boiling point is lower than normal (for a car cooling system). Namely, the car can boil over when the temp gauge is 'normal', or pretty near so. The temp gauge horizontal (9:00) is around 90C. Only 10 more degrees (C) and an unpressurized system boils. That's well before the red.
I would replace the cap. I would also look around for a bad hose or hose clamp, just in case. It may be tough, because things are probably pretty messy in the area.
