Disclaimer- I have gathered the following info. by spending lots of time reading this BB and its archives. Therefore, it qualifies as hearsay and doesn't result from personal experience. Hopefully, I'll be able to regurgitate this information to you in an accurate and factual manner.

The harmonic balancer (HB)consists of 4 metal pulleys sandwiched between rubber layers. 2 pulleys drive the water pump and alternator, 1 drives the power steering pump, and 1 drives the a/c compressor. If viewed from the side, it's kind of like this: HHHH where the "crossbar" of the H is the crankshaft, and the "thingies" (I think they're called serifs) in the corners are the edges of the groove for each belt.
When the rubber layers wear out, the pulleys start to develop some "play", resulting in slippage of the pulleys in relation to the crankshaft. Symptoms include squealing and noise, particularly at startup. I believe that the noise gets worse as the amount of wear increases, so there should be LOTS of warning before reaching the point of imminent failure.
To check, put a mark across the all of the pulleys. Make sure you mark the pulleys themselves, and not the belts. I think a chalk mark will work, some white-out or a paint pen might do the trick. The mark should be in a straight line between the engine and firewall.
After you hear the squealing, turn off the motor and find the mark you made earlier. If the pulleys have rotated in relation to the crankshaft (and each other), the HB is wearing out and the mark will no longer be in a straight line.
As far as replacement- honestly, I don't know enough to attempt an answer. I'm pretty sure that the job is considerably easier with the engine sitting on a workbench- the firewall might get in the way of removal.
