If there is no signal from the AMM, the engine control makes an assumption about airflow, and that assumption is to schedule fuel on the rich side. Running rich does make the engine idle nice. That doesn't mean the AMM is bad - it could be misadjusted, or it could be very correct, and there is some other problem in the system.
Yes, there is a burn-off requirement - I'm pretty sure it's just over 2000 rpm, and I don't think there is a coolant temp - but I'll check.
The most common symptom I see in a bad AMM is severe hesitation accelerating gently off the line, especially when cold. If you're stopped at a light, and gently press the gas pedal to accelerate away and the engine bogs down, that can be a sign of a bad AMM. If you press the gas pedal about 3/4 of the way down (stomp it), and the car takes off, then that's even more proof of a bad AMM. But a bad idle doesn't necessarily mean bad AMM.
