An AMM on the way out will start with wide swings at idle when cold that diminish as the engine warms up and the O2 sensor takes over. This progresses to hesitation problems and flat spots under acceleration regardless of temperature, and finally to chugging, coughing and black smoke. At the final stage, you may not want to run the engine long enough to warm up.
Try the AMM swap out method, but also look for air bubbles in the coolant expansion tank that become a steady stream when the engine is revved up. This would be a sure sign that you have a partially blown headgasket that is allowing coolant to get into a cylinder or two after the engine sits for awhile. This coolant burns off after a few minutes of very rough running. Also look for an oily film on the coolant surface and a frothy/cheesy goop on the oil dipstick. Finally check the spark plugs for one(two) that is extremely clean from the coolant being burned in its cylinder(the rest have a yellowish brown residue. A compression check or coolant pressure leakdown test will also show leaks.
