Generally happens during pad replacement. I try not to top off my brake fluid reservoir unless I replace pads. Unfortunately, if the car is run more than two years between pad replacements, the mc gets topped up during fluid flushes. In that case one should empty the mc before pushing/screwing the pistons back, or release the old fluid through the bleeders while pushing the pistons back. OTOH, I've also been know to simply gunk the engine and wash it after doing stuff that leaks brake fluid.....
