1. PCV fitting. There is a rubber bushing that holds it into the valve cover.
2. Over time, the bushings can shrink and cause a vacuum leak. Replacements are cheap from the sponsors. You may also have a valve cover gasket that's a bit loose causing a vacuum leak.
3. Opposite 3. It's a black plastic rectangular thing about 1/2' thick. The throttle shaft goes from the throttle bellcrank through the throttle body to the TPS.
4. No. A leak past that O-ring will just let unfiltered air into the system.
You may want to consider replacing all of the vacuum hoses and rubber bushings that hold the vacuum nipples into the intake manifold to eliminate a few potential vacuum leak sources.
