Will a bad cat cause a SAAB to run like a dog? (long)

My expensive quest to steady my idle has driven me nuts. I am dropping the car off to another mechanic tomorrow for diagnosis. He charges $74 an hour, so it better be good, or I am going to scream. Could a busted act cause a bad idle?
Also: Please let me know if the below is indicative of something you might suggest a solution for.
Problem: 25mpg. Missing (or sounding so) at idle. Sounds like a lawnmower. Strong at acceleration and cruise with some slight hesitation. On start the engine barely kicks into the higher rev that it used to initially. On cold start, it just might run pretty smoothly for a while. Maybe (hardlly ever). When the problem started, the car would always idle very well until warmed up. Now it's constant. It's been several months. Also: when coming out of gear, the engine drops VERY quicly down to 400 or so, then picks back up to hover around 750, maybe 1,000. Needless to say (perhaps not so) the car shakes at idle.
Replaced
Wires, plugs, amm, o2 sensor, head gasket, timing chain
Vacuum checked, valves reseated, distributor cap/rotor ok, switched out ezk, amm, knock sensor.
Any help would be greatly appreciated.
John
