one (#1)(in front of the throttle plate) should go to the bpv.
the other in front of the throttle plate (#2) should go to the aic, which then goes to the back side of the throttle plate (#3).
the last 2 (#4 and #5) are cooling lines. one is supply and the other is return. the crankcase does not actually vent to the throttle, it attaches down into the intake pipe between the amm and the turbo. right above the knock sensor, a small cooling line enters this line and parallels the crankcase vent line and then breaks off to go to the throttle (but the two mediums (crankcase air and coolant) never come into contact with one another). the other cooling line attaches to the metallic cooling line and the block.
coolant does go to the intake, but not into the intake. it's purpose is to heat the throttle body. i have heard reasons from heating the intake air for emissions to help the engine come up to temp quicker, to preventing icing in colder climates. not sure why, but many, including myself, have removed this line.
hope this helps.
