When your headgasket goes at about 150-250k miles, it is a good time to check the extension of the tensioner and the condition of the chain and sprockets. However, when my headgasket went at 240k, the chain tensioner was only extended about 2.5mm (less than 1/4)so on we go with the original chain. But I agree with davenew on replacement if engine is out at 200k+. Then you can easily replace the chain guides and lower sprocket at the same time.
The tensioner is allowed 11mm extension. Easiest to check tensioner when A/C compressor is moved out of the way for alternator repair, etc. Or you can just pull the valve cover and set the engine to TDC and check that the chain sprockets are lined up with the marks on the head. If you have chain noise, it is likely to be cause by a hardened damper pad that sits in the valve cover above the chain...easy 1/2 hour repalcement.
