The manual has a hard definition of the fuel to use, in terms of octane rating, which is better than 'regular'. But so much depends on the engine.
If you use low enough octane that the engine knocks, you'll get less performance and less power. If your engine only knocks under some rare conditions, like high load and hot days, you may not even notice.
Basically, if you use fuel with a high enough octane that your engine doesn't knock, you're getting maximum power and economy. Any higher, and you're wasting money. Any lower and you'll lose both.
Each engine is different. The rating on the fuel flap covers the greatest number of engines. As engines age, their fuel needs change. Some engine build up carbon and need higher octane. Some engines get looser and need lower octane. It depends on how you drive your car, altitude, and temperature.
So for two cars, one may need premium, and the other works fine on regular. There is no rule, other than you need to know what works for your car. Use the lowest octane that gives you full performance.
