Alternator - brushes on the voltage regulator are worn down to the service limit. You can get a new voltage regulator from on of the site sponsors (sobstory.com, eEuroparts.com, etc.) at a decent price. DO NOT get an Echlin voltage regulator from NAPA. It doesn't fit.
The usual way to fix the idle fluctuations like you describe is to first give the AIC valve a light rap with the handle of a hammer. If the fluctuations stop, then the AIC valve is bad. If they don't, you remove the AIC valve and give it a good cleaning inside with carb cleaner. Then clean the inside of the throttle body thoroughly with carb cleaner and something like an old, soft toothbrush.
If the vacuum hoses are old, you'll want to change them to eliminate vacuum leaks (which can also cause idle fluctuations.)
If you do all of that, you'll probably find that your idle has gotten much higher and you'll need to readjust the idle screw back to where it was before.
