Tricky vacuum leaks are often an issue there. Plus mis-timing somewhere along the chain. I posted not long ago about one of the spark plug wires not being snug against the plug and that caused my engine to fail. Even with a snug wire, perhaps the wire itself is faulty. A weakened or delayed spark in the ingnition cycle will cause a failed test. Also, got a new O2 sensor?
