Had similar problem. Like me you probably have several things that need replaced.
First make sure your boost (hooter) valve isn't leaking. If it hoots then its bad for sure. Thats a huge source for a vacuum leak, at least it was for me.
Look at the voltage at the O2 sensor. For the first ~thirty seconds it should be right at .5 volts + or - .02 volts. Then it should start to swing up and down from .2 to .8. Mine would start at .5 then start dropping down to less than .1 volts. That was the AMM telling the ECU to lean things out. (Bosch is supposed to be sending me an instruction package on LH which I'll share when I get it). I took the plug out of the amm and checked the resistance which was low as compared to the starting value as stated in bently. Setting it to 380 ohms with the adjustment screw made no difference. With the amm on the car I started adjusting the screw with the meter hooked up to the O2 sensor. It turns out that there is only one spot in 380 degrees of rotation of the adjustment screw where the AMM works. Anywhere else and the O2 sensor voltage starts to decay until it's ~.1 volt. A failed emissions test with very high NO emissions as the only problem confirmed that it was lean and the O2 sensor was working. I haven't had a chance to get back to have the emissions test done again but now car starts, idles nice runs great under any condition. Obviously I need a new AMM, but as long as it's working...
