You don't say what year/model car you have. With an older car, there is the possibility that the rubber bushing that surrounds the shift rod at the forward most point where it leaves the stick shift assembly unit and holds it in a centered position in relation to the shift assembly and tranny has become worn. If so, that adds a lot of unwanted additional play when shifting gears in a transmission that already has enough play. With the newer transmissions (88 and above), the center position spring inside the tranny helps remove a lot of the mystery for positioning the stick.
The plastic ball unit doesn't really wear out, but what can happen is the vertical alignment tab on the ball can shear away, which causes the stick to rotate. It is possible to tighten the ball unit if there's too much play. The metal bracket which screws down onto the ball can be bent so as to apply more downward pressure if tightening the screws do not remove the play enough.
