If you did a thorough flush 20K ago and it is black and smelly by now, something is going on. If the fluid is still red and clear I don't think you have a big problem.
No, you can't really put a "remaining time left" by doing a complete transmission service. You can get a hint of that doom is close or not likely, but it is other things that can kill the transmission instantly.
The two drive fingers on the converter that drives the oil pump can break of without warning and leave the car undrivable.
Sometimes it is possible to hear a grinding/rattling noise from the transmission oil pump area, but the only thing that tells you is that doom is very close because damage is already done to pump and drive fingers.
If you have 181K on the original auto box, you have pretty much exhausted the time left and every mile from now is a bonus. Just keep driving and give the tranny new fluid ones in awhile.
