If the idle is lower than normal it can cause misfiring, plus the engine is working against itself to keep turning. If your engine and head have lots of miles, all kinds of gunk can block air passages in the throttle body. Are all those vac line ports clear? Is the idle passage clear also? You might also want to look at that old distributor. Sometimes the rotating vacuum advance plate inside seizes from old dried up oil or rust, which causes the timing to be locked and ads to crappy running. Also, with an old engine like that, almost every intake component and hose connected to the intake side of the engine can develop a vacuum leak. The vac advance unit on the distributor for one. Does the engine have an EGR system? That valve can become gunked up and cause warm up problems. Consider dying spark plug wires as well. Bottom line: start with a thorough tune-up, by doing so you'll be eliminating common problem areas if the problem continues.
