Well, we've done it TWICE now. My car had two bad fenders when I bought it. Then last year I got in an accident with the car, and we did it again!
burlyb is right, you need to drill out all the spotwelds, then re-weld the new fender to the car. We got fenders from a local junkyard, had them cut out more than what was necessary to make sure we had all we needed. My car is a convertible, so it was even harder b/c we had to reweld the rollbar support (which runs down from the windshield frame).
This repair can last a long time if done right, much longer than the repair kits. What you really need is a solid fender.
As far as color, the second time we replaced my fender we found another edwardian grey 900, so the color matched. But since the damage was extensive and the new hood needed paint, we repainted the whole car anyway. So it can be done without having to paint it, yes.
If you're serious about doing it, and have questions, just ask. And search on the board, I think we've explained our experiences with it at least once before.
Good Luck!
