The crankshaft pulley (aka Harmonic balancer) is made up of three pieces. There are two pulley sections, with a rubber piece in the middle. Notice how there are two sets of belts down there? The rubber fails over time, and when you first start up, the two pulleys rotate at slightly different speeds. That slipping causes the squeal you hear. That spinning creates some heat and eventually the two pulleys spin the same speed - and the sound goes away.
Fixing this is critical - if the rubber fails completely, the pulleys will separate, and you'll lose your belts. No alternator, water pump, or power steering. The car will be undriveable.
How to check? Get under the car with a grease pencil or chalk. Draw a line across the pulleys. Start the car a few times when cold. Shut it off, and look at the line. If it isn't straight, then the pulleys are shifting.
