on the '89 and later I use a large waterpump or similar pliers, with the handles engaging the slots, and a bar stuck through to turn it. The bar tends to force the handles outward, so it is actually quite stable and allows applying a lot of force (good thing, you may need it!). And extra person really helps, to help hold everything together, and the bar should by lying flat on the deck rather than up higher since flex considerably wastes the effort if the bar is too high.
All the usual tricks like tapping around the top of the flange with a soft hammer to loosen the grip are well worth trying.
I just did a one of these pumps, and intending to get pictures of the whole procedure I could post, since this job has (with the later pumps) just about the highest profit/effort ratios of all (dealer wanted $700, I did it for $80/one hour) but alas my camera's memory card died and none of them came out!
