Disclaimer Warning: The following is entirely theoretical. Your prognosis does make sense as to why 2nd is popping out - if the fork was bent forward (I say forward since the action required to engage first gear is actually a reverse motion of the selector fork, causing the fork to bend to the front), then while engaged in 2nd, the spring detent on the selector rod isn't seated as securely since it sits that much out of center because the syncro gear can't seat any more into 2nd gear than it already is. I have seen the inside of an 89 tranny, but only through that side access panel. I can't imagine being able to do any repair given the confined space. One drastic attempt might be to somehow wedge something in between the fork and second and try and reverse the bend action by trying to force it into second. The good news there is since the selector rod on the tranny would have to be pushed forward in order to bend the fork back, you can use leverage against the end of the tranny selector rod with a large enough pry bar to do the job. Of course, in order to gain access to the end of that rod you need to seperate the stick shift rod from the tranny rod by removing the taper pin at the coupling. No easy task. Then you have to make sure the tranny rod is twisted clockwise (as looked at from inside the car) to engage the 1st and 2nd selector rod inside the box while wedging it forward. Otherwise you'd just be bending the fork for 4th gear. Of course, knowing just how much force should be used to bend the fork in the opposite direction than it is presently is unknown. If too much, then you have the opposite problem of 1st gear popping out.
