Like the other guys said, there are a few things that can contribute.
The first thing that comes to mind is that the shifter needs alignment. The pin that makes it necessary to pull up the collar to get into reverse is there to guide it into 5th. Chances are that the passenger side motor mount has collapsed, so it's necessary to move the shifter farther over to the right to get into 5th.
If you're a decent DIYer and don't mind getting covered with grease and grit, follow the link below to the Townsend Imports page on adjusting the shifter. Look it over and let us know if you feel comfortable doing the adjustment' Let us know if you do and we can give you a few hints that will make it go pretty easily.
Another possibility, especially if it's '92 or later, is that the shift coupler that connects the shift rod to the transmission, has started to come apart. '92 and later have a "dogbone" type of coupler that uses a rubber piece to connect two metal plates. (It's pretty obvious how it works when you get underneath the car and see it.) When the rubber deteriorates, it makes shifting a lot sloppier and harder to locate the gears. It's relatively easy to make a long-term temporary repair with hose clamps.
And, of course, there's nothing that says that it can't be a combination of reasons why you have the problem, including having the screws that hold the shift lever into the shifter assembly be very loose.
