1. Glass polishing.
If the lens is glass-made (commond silicious glass) I advise to buy a new one. Believe me - I go in for polishing (look at this pic and you'll copy what I'm talking about: http://new.ksenon.ru/img/catalog/merc_bixenn.jpg - have a look at usual MB headlights). If glass is sandblasted you'll need special tools and skills to perform its polishing.
But if the glass is "plactic" one (polycarbonate) it can be polished pretty easy with most common polishing tools and spreads.
2. Internal fogging.
2.1 Start with ventilation checking. Every headlight I've seen has a vent hole.
2.2 If you decide to perform disassembling and cleaning be VERY CAREFULL with reflector - it's very easy to demage its surface. And reflector is the main part in the headlight system to build the right and strong beam.
Simple donna touch reflectors surface. If it's muddy and you need to clean it - only sheet of water. Donna wipe it.
And remember that surface is pretty weak. As mechanically as well as chemically. Some mortars including some alcohols can destroy it very quickluy and easy.
