It is very rare with burned exhaust valves and especially on a non turbo.
Yes, it looks like a piece has broken off but it is most of the time burnt away.
It starts with a tiny breach of the seal between seat and valve and exhaust gasses start to flow throw pretty much creating a blow torch effect when they are squeazed through the tiny opening and the speed of the exhaust molecules are accelerating.
It doesn't take long for it to happen after the first breach of the seal.
It is usually created by a lean running condition from either a vacuum leak or a fuel injector with uneven spray pattern.
It also usually happens when the car is driven hard by towing something or is going up a long incline.
