Check the rubber shift linkage coupling. If it's not going fully into gear, due to a bad coupling, then it will pop out of gear. As the rubber part of the coupling separates it will often still work OK when pushed or twisted one way but not when pulled or twisted the other way. That's why only some gears are affected. Having said that, your reverse problem could well be solved by doing the "reverse gear fix" where you turn the idler engagement arm around or (as I do) heat and bend it, thus increasing the depth of engagement. There are plenty of posts here on that topic (search "reverse gear fix"). You could have bad motor mounts and that will sometimes cause shifting problems but check the coupling first. A failed motor mount will let the engine (and tranny) rise up when you apply a little gas with the car in gear and the brake pedal on the floor. Could also be that you just have some internal tranny problems, rebuilt or not, but don't think in those terms until you eliminate the easy, external possibilities.
