Smacking the cat around will break up the substrate and send it shooting through the rest of the exhaust. My guess is that your issue is not related to the catalytic converter. If the car has a CEL after starting, at idle, you've got something wrong and you're stuck in limp home mode - which might prevent the engine from revving over 2k. Most likely, there is something wrong with the air mass meter, but it could be ignition related (bad combustion, bad rpm signal). The surging at startup is consistent with a bad AMM, FWIW.
