OK, I figured it out. Posting the answer here so that it is searchable, the next time somebody tackles this.
Removed the front carpeting, the knee bolster and large lateral air duct. Did not have to remove the seats, as I could get under the rear carpeting with a torx bit and a small 1/4" socket driver.
The longitudinal ductwork is glued somehow to the side rubber backed foam pads that retain water. Was able to work the rear loose, remove the two top screws; I dimly and finally realised that there was glue involved.
The original heat shield has a stud on top, not the washer with a hole. The original nut was a (large) 13mm. To get even a small 10mm head nut and bolt, I had to drill the washer out some. So the replacement shield is a bit different, but at least the holes lined up well; installation was only about fifteen minutes...and I'm slow.
Finally the new shield lined up perfectly and looked great! I'm planning to remove it at some point
