A clutch failing to fully release might mean hard shifts, but most likely would show up in first gear, and reverse, first. Once you're under way shifting is easier and synchros help, so a clutch not entirely released isn't likely to cause your 4th gear problem.
As Hans said it sounds as if throwout bearing froze and wore thru pressure plate fingers.
You could throw new clutch components in and see how it does. Car otherwise okay?
