So the rust hole was as I suspected. Rusted completely through but only a few inches wide. In the body shop now having all the cancer cut out and new metal put on.
I'm not going the welding route, as that would definitely be prohibitively expensive, but rather I will have fresh sheet metal laid over the hole and secured using industrial adhesive made for this application, and undercoating sprayed underneath. That's what the body shop guy recommended and it's also what I did on the drivers side (see pic below for driver's side repair). Then I'll POR15 the inside, lay on sound deadener and new foam, and voila!
