At 250K it's very likely that some bearings in the unit are shot and causing slop that interferes with shifting under certain conditions. Best to deal with it before a gear breaks, the supply of good used ones gets smaller every day and of course there are no new ones. If the linkage is sloppy it can't hurt to address that and see what you get. Sloppy linkage (a damaged rubber coupling usually) will make for tricky shifting.
