Two possiblities come to mind - The first is that the thermal switch mounted in the radiator has failed shut. The other is the fan relay has failed closed. My first guess would be the fan relay, mostly because you could check it. Pull the relay out and measure across contacts 30 and 87. If it shows continuity, the relay is bad.
If you can get access to the thermoswitch contacts, check continuity there with a cold car, engine off. If it shows continuity, the switch is bad.
